Physical Forms 2025-2026

Please print these out and take to your doctor or urgent care for them to fill out. All athletes will need to have a physical in order to tryout for a sport. For both middle and high school sports, the physical must be done after March 1st of 2025. If the physical was done prior to the 1st of March, we will not accept the physical and they will not be allowed to play until a new physical is provided. 

 

Once the physical is completed, please upload ALL PAGES on their Big Teams profile. WE WILL NOT ACCEPT PHYSICAL COPIES.
